Refer a Young Adult
Help a young person find their footing.
Freedom House partners with case workers, social workers, and referral agencies across Colorado to connect eligible young adults with stable housing and a supportive community.
Who we serve
Freedom House provides transitional housing for young adults ages 18–24 who are aging out of foster care or transitioning out of residential treatment programs. We serve young people who have demonstrated motivation and readiness to take the next step toward independence — and who would benefit from a structured, community-oriented environment to do it.

How to Refer
The referral process, step by step
Reach Out
Email us at shannon@freedom-house.info to introduce yourself and your client. Include a brief overview of their situation and why you believe Freedom House is a good fit.
Initial Conversation
We'll schedule a call to learn more about the youth's background, goals, and readiness. This helps us determine fit and answer any questions you or your client may have.
Youth Interview
If there's a strong initial fit, we'll invite the young adult to connect with us directly — so they can ask questions, see the space, and make an informed decision.
Placement Decision
We'll work with you to determine next steps. If placement is a good match, we'll coordinate move-in details and ensure a smooth transition.
